Measuring and Understanding the Ankle Brachial Index (ABI) The Ankle Brachial Index (ABI Test) is an important way to diagnose peripheral vascular disease. The index compares the systolic blood pressures of the arms and legs to give a ratio that can suggest various severity of peripheral vascular disease.
How to perform the ABI. Measurement of the ABI can be easily performed in a clinician’s office using a blood pressure (BP) cuff and handheld Doppler device with a vascular probe. Systolic BP is determined in both arms and both ankles. An ABI measurement can usually be performed in less than 10 minutes. Risk Factors for PAD
Inflate the cuff to about 20 mmHg above the expected systolic blood pressure of the patient. The Doppler signal should disappear. Then slowly deflate the cuff, approximately 1 mmHg/sec. When the Doppler signal re-appears, the pressure of the cuff is equal to the brachial systolic pressure.
Carser 41 claims that the normal corrected ABPI range is 1.02 to 1.78 (admittedly, Carser was studying ABPI ranges in the context of background hypertension). Of 100 studies of ABPI methodology included in their review, Klein and Hage 42 found that the most frequently quoted normal value for ABPI was 1.0.
