Sep 13, 2007 . When using an apex locator on a tooth having a metallic restoration (PFM, gold crown, or a large alloy), the apex locator will often give an inaccurate reading. An easy solution is to insulate the file from the metal filling or crown by placing a plastic bur sleeve just below the rubber stopper on the file. This is a great trick!

Make sure that you are getting your initial reading by taking your file a little long to read the PDL, then back up your file short of working length, then re-advance it to your zero zero. This will “calibrate” your apex locator and will give you a more accurate reading. If playback doesn't begin shortly, try restarting your device.
The apex locators are attached to the patient's lip on one side and the other side is attached to the file. The file is circuit. ( 5) walls of the root canal due to the presence of the transparent d entin. The tooth exhibits increa sing electrical impedance across the walls of the root canal, which is greater apically than coronally.
Tip #3: Pull your file away from metal fillings or crowns before taking your reading. When your apex locator is in use, make sure your file is away from the sides of the tooth, any metal fillings or crowns. Again, if your apex locator is chirping at you, something’s up!

The first generation (resistance based) apex locators use this value to determine the location of the apical constriction. Unfortunately, they were unreliable and would short-circuit when the file contacted pulp tissue, exudate, irrigating solutions or blood in the canal.
A recent systematic review, which includes in-vivo studies found apex locators could safely be used in patients with CIED. It is recommended to keep the EAL at least 10-20cm away from the CIED and its leads. Consultation with the patients cardiologist remains advisable.
