The main difference between the pacemaker and the defibrillator is the addition of an electrical shock feature that is designed to send current to the cardiac muscle when the heart rate increases to a dangerous level.

The pacer is there to do whatever we need, however often we need it. The heart is a muscle. After the leads are put in, it heals just like any other injured muscle. The signal the pacer puts out is very minimal, mimicking what our heart should have been doing on its own so it does not damage the heart.
When you need a defibrillator. You need an ICD if you have ventricular fibrillation (VF). That’s when the heart’s lower chambers don’t contract hard enough to pump blood throughout the body, triggering cardiac arrest. You also need one if you’ve already suffered cardiac arrest, from VF or another cause.
