Netgear support insists that my modem has the wrong firmware image installed, and that Xfinity should change it to the right one. Appears my v2 modem may be running v1 hardware firmware (v6.01.06), but should be running firmware V1.01.03. My modem info: CM1000v2 Hardware Version 2.02 Firmware Version V6.01.06

Your downstream power of 4.5 to 6.5 dBmV is a bit on the high side. My CM1000v2 power is in the range -1.1 to 0.7 dBmV. If you have a 3dB or 6dB coax cable attenuator handy you could try lowering the signal a bit. Do you see Downstream OFDM channel locked?
