Research Topics Patient-Aligned Care Teams (Pact) Pact is a team-based model of care in which a team of health professionals, led by a provider, works collaboratively with the patient to provide for all of the patient's healthcare needs—or appropriately coordinates care with other qualified professionals.
Pact serves up to 1,200 people statewide with effective and intensive outreach services. These services are evidence-based, recovery-oriented, and provided through a team approach. With small caseloads, Pact teams can address each person's needs and strengths to provide the right care at the right time.
Reduce the need for care within state hospitals. Increase satisfaction and quality of life. Decrease the use of community inpatient and crisis services. Increase employment. Reduce involvement with criminal justice. To enhance Pact services, the legislature mandated that HCA stand up a total of eight new Pact teams.
To receive Pact funding, apply, register for classes (12+ credits), and fill out the FAFSA. Entry into the program will be accepted on a first-come first-serve basis. How to Qualify? Get Started FAQs Help How to Qualify? Am I eligible for Pact if I took college courses in high school?
