  • How is a patient referred to the PD Program?

    If there are no absolute contraindications, the PD team is made aware of the patient’s choice and the patient is referred to the PD program. The patient is commonly referred to the PD program by a nephrologist but may also be referred from the current modality care team or nurse navigator.

  • What should I do if I have a PD bag?

    Safely dispose of bags and other medical waste materials. Connect and disconnect your catheter to a PD bag in a sterile way to prevent infection. Take daily vital signs and weigh yourself. Add medicine to your PD bag if you need to. Troubleshoot issues involving your treatment.
