Transitional Care Management. The purpose of Transitional Care Management is intended to improve the patient’s care post- acute discharge and ensure the patient is transitioned back into the ambulatory setting with their primary care or specialist provider and their likelihood for readmission/rehospitalization is lowered.
Transitional care management is an important piece of the puzzle for monitoring and managing chronic conditions. It helps ensure that the patient's needs are being met during the transition from inpatient care to the patient's community setting, reducing the risk for relapse and readmission.
Chronic care management (CCM) services are generally non-face-to-face services provided to Medicare beneficiaries who have multiple (two or more) chronic conditions expected to last at least 12 months, or until the death of the patient.
